DVA Gold and White Card holders welcome at ZH Doctors Cleveland. Coordinated Veterans’ Care (CVC) with GP and nurse support in Cleveland.

ZH Doctors Cleveland is proud to care for veterans and their families. We welcome Department of Veterans’ Affairs (DVA) Gold Card and White Card holders, bill DVA directly for eligible services, and offer the Coordinated Veterans’ Care (CVC) program at all ZH Doctors Network clinics.
Please bring your DVA card to every appointment.
CVC is a DVA program that helps veterans manage ongoing (chronic) health conditions, improve their quality of life and reduce the chance of unplanned hospital stays. Your GP works with a care coordinator, usually our practice nurse, to create a personalised care plan and keep in regular contact with you, in person and by phone.
You must also live in Australia, agree to a care plan and meet the criteria your GP assesses. People living permanently in residential aged care are not eligible.
Care coordination under CVC is free for eligible veterans.
Do you bulk bill DVA patients?
Yes. ZH Doctors Cleveland bills DVA directly for eligible services, so Gold Card holders have no out-of-pocket cost for GP consultations.
Is the CVC program available at every clinic?
Yes. The Coordinated Veterans’ Care program is offered at all ZH Doctors Network clinics.
How do I join the CVC program?
Book an appointment with your GP and tell our reception team you are a DVA card holder interested in CVC. Call us on 07 2142 4233.
