Chronic disease management at ZH Doctors Cleveland is bulk billed for eligible patients with a valid Medicare card. Living with a long-term condition such as diabetes, heart disease, asthma or arthritis can be challenging, but you don’t have to manage it alone. Our GPs work with you to create a personalised care plan that helps you stay well and prevent complications.
What is chronic disease management?
A chronic condition is one that has been, or is likely to be, present for six months or longer. Under Medicare, your GP can prepare a GP Chronic Condition Management Plan (GPCCMP), which replaced GP Management Plans and Team Care Arrangements from 1 July 2025. Your plan sets out your health goals, the treatment and support you need, and the steps you and your care team will take together.

Medicare-subsidised allied health services
